青少年与成人首发抑郁障碍患者临床特征及颅脑 磁共振成像结构图像异常特征比较论文

【摘要】   目的:比较青少年与成人首发抑郁障碍患者的临床特征及颅脑磁共振成像(MRI)结构图像异常特征,为青少年首发抑郁 障碍发病机制的探讨提供依据。方法: 回顾性分析 2017 年 7 月至 2018 年 12 月首都医科大学附属北京安定医院收治的 167 例青少年首发 抑郁障碍患者的临床资料作为青少年组,将同期收治的 198 例成人首发抑郁障碍患者的临床资料作为成人组,比较两组临床特征及颅脑  MRI 结构图像异常特征,并采用 Logistic 回归分析青少年首发抑郁障碍患者伴精神病性症状的影响因素。结果: 青少年组母孕期异常及伴 精神病性症状占比均高于成人组, 差异有统计学意义( P<0.05); 两组性别、家族史阳性、性格特征、病程、有无自杀观念分布情况比较, 差异均无统计学意义( P>0.05) 。青少年组颅脑 MRI 结构图像异常 78 例(46.71%), 成人组颅脑 MRI 结构图像异常 95 例(47.98%), 两组比较,差异无统计学意义( P>0.05) ;其中,两组海马萎缩占比比较,差异无统计学意义( P>0.05) ;青少年组皮质萎缩占比和其他 颅脑 MRI 结构异常占比均低于成人组, 先天性脑发育异常占比高于成人组, 差异有统计学意义( P<0.05) 。青少年首发抑郁障碍伴和不伴 精神病性症状患者的性别、年龄、起病年龄、病程、母孕期异常、家族史阳性、性格特征、自杀观念、海马萎缩分布情况比较,差异均无 统计学意义( P>0.05) ;青少年首发抑郁障碍伴精神病性症状患者先天性脑发育异常占比高于不伴精神病性症状患者,差异有统计学意义 ( P<0.05);经Logistic 回归分析结果显示, 先天性脑发育异常为影响青少年首发抑郁障碍患者伴精神病性症状的危险因素( OR>1,P<0.05)。 结论: 青少年首发抑郁障碍患者伴精神病性症状占比高于成年人首发抑郁障碍患者,青少年首发抑郁障碍患者皮质萎缩占比和其他颅脑  MRI 结构异常占比低于成人首发抑郁障碍患者,先天性脑发育异常为影响青少年首发抑郁障碍患者伴精神病性症状的危险因素。

Comparison of clinical features and abnormal features of structural brain magnetic resonance imaging in adolescent and adult patients with first-episode depression

【Abstract】 Objective: To compare clinical features and abnormal features of structural brain magnetic resonance imaging (MRI) in adolescent and adult patients with first-episode depression. Methods: The clinical data of 167 adolescent patients with first-episode depressions admitted to Beijing Anding hospital affiliated to Capital Medical University from July 2017 to December 2018 were retrospectively analyzed and they were set as adolescent group, and 198 adult patients with first-episode depression treated during the same period were taken as adult group. The clinical features and the abnormal features of structural brain MRI image were compared between the two groups. Logistic regression was used to analyze the influencing factors of psychotic symptoms in the adolescent patients with first-episode depression. Results: The proportions of abnormal maternal pregnancy and psychotic symptoms in the adolescent group were higher than those in the adult group, and the differences were statistically significant (P<0.05). There were no significant differences in gender, positive family history, personality characteristics, course of disease, and the distribution of suicidal ideation between the two groups (P>0.05). There were 78 cases (46.71%) of abnormal structural brain MRI images in the adolescent group, and 95 cases (47.98%) in the adult group with abnormal structural brain MRI images, and the difference was statistically significant (P<0.05). There was no significant difference in the proportion of hippocampal atrophy between the two groups (P>0.05). The proportions of cortical atrophy and other structural brain MRI abnormalities in the adolescent group were lower than those in the adult group; the proportion of congenitalencephalodysplasia was higher than that in the adult group; and the differences were statistically significant (P<0.05). There were no significant differences in gender, age, age of onset, course of disease, abnormal maternal pregnancy, positive family history, personality characteristics, suicidal ideation, and distribution of hippocampal atrophy among the adolescent patients with first-episode depression with and without psychotic symptoms (P>0.05). The proportion of congenital encephalodysplasia in the adolescent patients with first-episode depression with with psychotic symptoms was higher than those without psychotic symptoms, and the difference was statistically significant (P<0.05). Logistic regression analysis showed that congenital encephalodysplasia was a risk factor affecting the adolescent patients with first-episode depression with psychotic symptoms (OR>1, P<0.05). Conclusions: The proportion of the adolescent patients with first-episode depression with psychotic symptoms is higher than that of the adult patients with first-episode depression. The proportions of cortical atrophy and other structural brain MRI abnormalities in the adolescent patients with first-episode depression are lower than those in the adult patients with first-episode depression. Further, congenital encephalodysplasia is the risk factor affecting the adolescent patients with first-episode depression with psychotic symptoms.

青少年首发抑郁障碍发生率约为 12%,给患者 本人、家庭及社会带来了沉重负担 [1-2] 。青少年首 发抑郁障碍与成人首发抑郁障碍诊断标准相同,但 越来越多的证据表明两种疾病在抗抑郁药的疗效和 临床表现方面存在差异:治疗上青少年首发抑郁障 碍患者较成人首发抑郁障碍患者的效果差 [3-5] ;且 青少年首发抑郁障碍患者食欲减退、体质量减轻、 精力丧失及失眠占比较成人首发抑郁障碍患者高, 而性欲下降及注意力减退在成人首发抑郁障碍患者 中多见 [6] 。提示青少年首发抑郁障碍与成人首发抑 郁障碍在神经病理基础上可能存在差异。磁共振成 像(magnetic resonance imaging,MRI) 是研究抑郁 障碍患者大脑结构和功能的重要工具 [7-8] 。已知青 少年首发抑郁障碍患者的多个脑区在执行情感处理 任务时异常激活,可能与临床症状相关 [9-10]。在结 构 MRI 方面,青少年首发抑郁障碍患者背内、外 侧前额叶皮质和左侧前扣带回皮质尾部变厚 [11-12]。 而成人首发抑郁障碍患者全脑皮质厚度与健康人无 明显差异 [13] 。上述研究表明青少年首发抑郁障碍 患者与成人首发抑郁障碍患者在临床症状及脑形态 学方面的特征不同。本文比较青少年与成人首发抑 郁障碍患者的临床特征及颅脑 MRI 结构图像异常 特征,为青少年首发抑郁障碍发病机制的探讨提供 依据。

1  资料与方法

1.1  一般资料  回顾性分析 2017 年 7 月至 2018 年 12 月首都医科大学附属北京安定医院收治的167 例青少年首发抑郁障碍患者的临床资料作为青 少年组,将同期收治的 198 例成人首发抑郁障碍 患者的临床资料作为成人组。纳入标准:青少年 首发抑郁障碍患者年龄≤ 18 岁,成人首发抑郁障 碍患者年龄 >18 岁;符合《精神与行为障碍分类 临床描述与诊断要点(国际疾病分类第 10 版) 》 中抑郁障碍的诊断标准;首次发病;住院期间行颅脑 MRI 检查;临床资料完整。排除标准:符合 器质性精神障碍诊断标准;符合物质滥用或物质 依赖诊断标准;头部外伤。青少年组:平均年龄  (14.89±1.47)岁, 平均起病年龄(13.76±1.45)岁。 成人组: 平均年龄(42.75±12.49) 岁, 平均起病 年龄(41.60±12.62)岁。

1.2  方法

1.2.1  资料收集  收集青少年首发抑郁障碍患者和 成人首发抑郁障碍患者的临床资料及相关特征信 息, 包括性别、母孕期异常情况、家族史阳性情况、 性格特征、病程、有无自杀观念、是否伴有精神病 性症状(如幻觉、妄想)等。

1.2.2  MRI 检 查  采 用 西 门 子 MRI 成 像 系 统   MAGNETOM Prisma 3.0T, 标准 20 通道正交头线圈 进行 MRI 扫描。检查前告知患者扫描流程、大致 需要时间及注意事项。然后进行轴位 [T1 加权成像  (T1WI)、T2WI、T2 液体衰减翻转恢复成像(T2Flair)、 扩散加权成像(DWI)]、矢状位(T2WI)及冠状 位(T1WI、 T2WI)序列扫描。由 2 名放射科医师对 所有颅脑 MRI 结果进行盲法阅片,分类、整理颅 脑 MRI 结构图像异常表现。

1.2.3  颅脑结构异常的 MRI 诊断依据  将青少年 首发抑郁障碍患者和成人首发抑郁障碍患者的颅脑 MRI 结构图像特征分为 4 类,包括皮质萎缩、海马 萎缩、先天性发育异常和其他。其中皮质萎缩为除 海马萎缩外的皮质萎缩,包括额叶萎缩、颞叶萎缩 和顶叶萎缩;先天性发育异常包括大枕大池、静脉 发育不良、颅内囊肿、脑容积缩小和脑室扩张;其 他包括腔隙性脑梗死、缺血灶和小脑萎缩。

具体诊断标准 [14-16] : (1) 皮质萎缩。皮质体积 缩小,灰质变薄,脑表面脑沟及脑池扩大,脑室大 小正常,见图 1、2。(2)海马萎缩。海马体积缩 小, 侧脑室颞角扩大, 邻近脉络膜裂增宽, 见图 3。

(3)脑容积缩小。18 岁以下出现的全脑脑沟增宽、加深,脑回变窄,外侧裂池及蛛网膜下隙增 宽。(4)脑室扩张。脑室扩大,三脑室亦可扩大, 脑沟、脑池大小正常。(5) 静脉发育畸形。MRI 结构图像表现为许多细小扩张的髓静脉呈放射状 汇入一条或多条粗大的引流静脉。(6)颅内囊肿。 MRI 结构图像呈脑脊液信号影,位于颞底、半球凸 面及后颅凹等,邻近脑组织移位,局部颅骨变薄, 增强扫描时囊壁无强化。(7)大枕大池。小脑后 方蛛网膜下隙呈囊性扩张,其内为脑脊液样信号, T1WI 呈低信号,T2WI 呈高信号,其邻近枕骨略有 变薄,见图 4。(8)腔隙性脑梗死。T1WI 呈低信 号、T2WI 呈高信号, 病灶大小为 3~20 mm。(9) 缺血灶。脑内皮层下白质可见单发及多发班点状异 常信号影, T1WI呈等信号, T2WI及T2Flair呈高信号, DWI 呈等信号改变。(10)小脑萎缩。脑桥池、桥 小脑角池、枕大池、小脑沟及四脑室扩大。

1.3  观察指标  (1)比较两组临床特征。(2) 比较两组颅脑 MRI 结构图像异常特征。(3)影响 青少年首发抑郁障碍患者伴精神病性症状的单因素 分析。(4) 影响青少年首发抑郁障碍患者伴精神 病性症状的 Logistic 回归分析。

1.4  统计学方法  应用 SPSS 20.0 软件进行统计 学分析,计量资料以(   ±s )表示,采用 t 检验  ( n<100) 或 u 检验( n>100) ,计数资料以率(%) 表示,采用 χ2 检验,多因素分析采用 Logistic 回归 分析,以 P<0.05 为差异有统计学意义。

2  结果

2.1  两组临床特征比较  青少年组母孕期异常及 伴精神病性症状占比均明显高于成人组,差异有统 计学意义( P<0.05) ;两组性别、家族史阳性、性 格特征、病程、有无自杀观念分布情况比较,差异 均无统计学意义( P>0.05)。见表 1。

2.2  两组颅脑 MRI 结构图像异常特征比较  青少 年组颅脑 MRI 结构图像异常 78 例(46.71%), 成 人组颅脑 MRI 结构图像异常 95 例(47.98%), 两 组比较, 差异无统计学意义( χ2=0.059, P=0.808)。 其中,两组海马萎缩占比比较,差异无统计学意 义( P>0.05) ;青少年组皮质萎缩占比和其他颅脑 MRI 结构异常占比均明显低于成人组,先天性脑 发育异常占比明显高于成人组,差异有统计学意义 ( P<0.05)。见表 2。

2.3  影响青少年首发抑郁障碍患者伴精神病性症 状的单因素分析  青少年首发抑郁障碍伴和不伴 精神病性症状患者的性别、年龄、起病年龄、病 程、母孕期异常、家族史阳性、性格特征、自杀 观念、海马萎缩分布情况比较,差异均无统计学 意义( P>0.05) ;青少年首发抑郁障碍伴精神病性 症状患者先天性脑发育异常占比明显高于不伴精 神病性症状患者,差异有统计学意义( P<0.05)。 见表 3。

2.4  影响青少年首发抑郁障碍患者伴精神病性症 状的 Logistic 回归分析  以是否伴有精神病性症状 为因变量,以是否伴有先天性脑发育异常为自变量 (0= 无,1= 先天性脑发育性异常,2= 非先天性脑 发育异常) 。经 Logistic 回归分析结果显示,先天 性脑发育异常为影响青少年首发抑郁障碍患者伴精 神病性症状的危险因素(β=0.845, Wald/χ2=5.459, P=0.019, OR=2.328,95%CI 1.146~4.731)。

3  讨论

本研究结果显示,青少年组母孕期异常及伴精 神病性症状占比均高于成人组,这一结果与文献报 道相吻合 [17]。提示母孕期异常与精神疾病的发病率 增高有关。已知伴有精神病性症状的抑郁障碍是抑郁障碍的一个亚型,抑郁症状一般较严重,其在发 病机制上可能有独特的病理基础 [18]。而额叶和岛叶 皮质的结构和功能变化较情绪处理区的变化更能区 分伴或不伴精神病性症状的抑郁障碍,与调节情绪 环路的皮质异常无关 [19]。本研究结果同时显示, 青 少年组皮质萎缩占比和其他颅脑 MRI 结构异常占 比均低于成人组,先天性脑发育异常占比高于成人 组;青少年首发抑郁障碍伴精神病性症状患者先天 性脑发育异常占比高于不伴精神病性症状患者,且 进一步经 Logistic 回归分析结果显示,先天性脑发 育异常为影响青少年首发抑郁障碍患者伴精神病性 症状的危险因素。提示先天性脑发育异常与伴精神 病性症状相关,同时伴精神病性症状的抑郁障碍患者与不伴精神病性症状的抑郁障碍患者的大脑病理 改变基础不同。

本研究结果还显示,两组海马萎缩占比比较, 差异无统计学意义,提示海马萎缩可能是抑郁障碍 患者的共同脑结构改变,这一结果与文献报道相吻 合 [20-21] ,但此结论仍需进一步验证。未来研究中, 关于伴精神病性症状的抑郁障碍与大脑结构改变的 关系,可以结合基于体素的形态学分析及结构网络 等技术对大脑结构进行详细分析,为进一步阐明伴 精神病性症状抑郁障碍的发病机制提供参考依据。

此外,虽然本研究中青少年首发抑郁障碍患者 未发现额叶、颞叶皮质萎缩,但既往研究发现青少 年额叶和颞叶皮质变薄是成人首发抑郁障碍患者的 易感指标 [22],  其结果尚待采用队列研究进一步阐明 皮质萎缩与抑郁障碍发生的相关性。因本研究是关 于青少年首发抑郁障碍的初步研究,未来仍需进行 队列研究,延长随访时间,观察青少年时期起病的 成年期患者的脑结构情况及与治疗效果的关系,以 进一步阐明青少年首发抑郁障碍的发病机制和危险 因素,为青少年首发抑郁障碍的早期识别及干预提 供依据。

综上所述,青少年首发抑郁障碍患者伴精神病性症状占比高于成年人首发抑郁障碍患者,青少 年首发抑郁障碍患者皮质萎缩占比和其他颅脑 MRI 结构异常占比低于成人首发抑郁障碍患者,先天性 脑发育异常为影响青少年首发抑郁障碍患者伴精神 病性症状的危险因素。
